I understand your risk management comments. It is so important to get accurate data, and then of course utilize it properly. I once had to investigate why the impossible happened at work. 
Essentially it was caused by a person not accepting the data that was presented, and a made flawed decision , and passed it on, ( as fare as I am concerned he deliberately lied), that resulted in situation where some individuals panicked , thus the impossible happened. 
I have come to the conclusion that common patterns can occur, in many different areas, technology, 
commerce, the climate , and so on. 
If nations, departments, and people are fed false information by the media and the administrations, they then can be confronted with a tipping point where control is lost and the impossible happens. 
I think 9/11 falls into that category, climate change is another , and what of the travesty of Iraq , 
where are the garlands of flowers, and folks taking leisurely walks in the parks? Its out of control. 
Afghanistan is a lost cause to the so called allies, (or what ever they call themselves), the truth is not being told I am sure , and many people in the west will not understand why their nations are caught up in a perpetual war.
